The program is free to all CMGMA members and includes continental breakfast, networking with peers, the creation of study groups, and core curriculum materials for the Certification and Fellowship process. Further educational materials will be available for purchase from MGMA.
We will begin the morning with an informational session on the Certification process, including the Scholarship fund. We will also review the "Body of Knowledge" which is the core curriculum for the certification process. Then beginning at approximately 10:00 am, we will begin discussing the fellowship process which will include a Tutorial packet of information on the Fellowship process.
We hope that this informational session will be the kick start that CMGMA members need to get moving toward their certification and fellowship goals. Join us for our Annual "Healthcare Year in Review" and Annual Meeting.
Each year, CMGMA conducts a Year in Review in Healthcare. This meeting is designed to give practice managers an update on important laws, regulations, and initiatives that impact the daily operations of your practice. From changes to PA supervision, CME requirements, professional rehabilitation assistance programs, HMO litigation, a medical liability update, and informed consent, Jennifer Cox will discuss the ins and outs of this year's major State law changes. She will also discuss three other hot topics: 1. HIPAA security audits (the Government is on the move); 2. Understanding EMR/IT sharing rules including fraud and abuse and tax considerations; and 3. Possible downstream ramifications from the growth of no-pay quality indicators.
After our Annual Meeting, at which the 2008 Board of Directors will be elected, Lisa Goldstein from MGMA will present the “Washington Update” which will include timely information on the status of Medicare administrative simplification, Medicare reimbursement and fraud and abuse compliance. The presentation will also address other hot topics under consideration by Congress and federal regulatory agencies.

Join us for our Annual "Healthcare Year in Review" and Annual Meeting where we will elect our 2009 Board of Directors.
Each year, CMGMA conducts a Year in Review in Healthcare. This meeting is designed to give practice managers an update on important laws, regulations, and initiatives that impact the daily operations of your practice. This year, Attorney Jennifer Cox will provide an information management overview including a HIPAA and state confidentiality update; new consumer protections, including how you must protect Social Security numbers and follow the federal “Red Flag” rules for patient financial information; an update on the planned shift from ICD-9 to ICD-10; eprescribing and tamper proof prescriptions; and evolving EMR/EHR rules and plans. Jennifer will also present a Stark overview and updates including how Stark works, what to look for and why practices should care. Also included in Jennifer’s presentation will be an explanation of no pay events and a pay for performance update. Jennifer will conclude her presentation will a legal round up including new case law; new legislation and regulations affecting medical practices; and a discussion of possible new laws for the coming legislative session.
In the afternoon, participants will be invited to join round table discussions with other practice managers to discuss issues of importance to their specialties and practices.

In March 2006, when Chuck Stohler and Nick Zaino first presented to CMGMA, the positive feedback was overwhelming with most attendees requesting another session. This year, Chuck and Nick will review and provide an update on some topics discussed at last year’s meeting such as hiring and interviewing; discrimination and harassment issues; documenting performance issues; workplace investigations; disability/absenteeism issues; and wage payment questions. Nick and Chuck will also lead a discussion on electronic communications in the workplace and other “hot” developments in labor and employment law. They will devote a significant portion of the program to answering questions.
There will also be time at the end of the program for questions and answers.

Join us for our first program of 2008 where we will be joined by Max Reiboldt of the Coker Group. During our morning session, Mr. Reiboldt will present “HSAs, HRAs, and Other High Deductible Health Plans: How They May Affect Patient Behavior and The Practice.” Then, in the afternoon, Mr. Reiboldt will build on his morning presentation with "Going Retail – Strategies to Grow beyond Traditional Practice Models”.
As practice managers know, the rapid onset of consumer choice models coupled with high deductible insurance plans give patients more control over their health care spending. Consumer-driven health plans will compel practice mangers to look at accounts receivable management as well as the role of physician and business office and how these issues may be affected by these health plans and benefits. Patients who want to be seen may hesitate to get the care that they need due to the out-of-pocket expense. How will patients handle being in total control of where their healthcare dollars go, what will their priorities be and will other resources will be utilized for their healthcare needs? An increasing number of employers are considering adopting HSAs or other high deductible health plans.
From the employer’s perspective, this program gives insight to practice administrators for analyzing this option and provides steps for successful transition from traditional plans to HDHPs. From the provider’s viewpoint, practice administrators must be ready to make “deals” in the consumer-driven environment. Some patients may want to negotiate your fees, while others will defer needed care due to high out-of-pocket expense. The rise in popularity of the new models will compel you to amend billing and collection policies.

Webinar on OIG Work Plan and RAC Audits: What you Need to Know to be Prepared in 2010
CMGMA is offering a free webinar which will focus on the 2010 OIG Work Plan as it relates to physicians. Join practice administrators, billing and coding staff, compliance officers and physicians who rely on the OIG’s annual report to review risk areas in Part B Billing. Nancy Enos will discuss the latest and most pressing compliance concerns for physicians in the OIG Work Plan. You’ll walk away with a strong understanding of the OIG’s key concerns, as well as best practices for staying in compliance.
This seminar will also provide an update on RAC audits. The Medicare Recovery Audit Contractor (RAC) program is charged with identifying improper Medicare fee-for service payments, both overpayments and underpayments. In October 2008, the Centers for Medicare & Medicaid Services (CMS) named four permanent RACs nation wide. What does this mean to you as a Medicare provider? The RAC may contact you to request medical records. The records will be used to assist the RAC to identify Medicare overpayments and underpayments. In addition, the RAC may communicate review findings and overpayments determined through their efforts. This session will review the number of records, and frequency of RAC record requests for your practice.
The Consultation Coding changes for Medicare will also be discussed, including important billing implications and crosswalk samples.

This year's September Educational Program will feature Frank Cohen, MPA, a certified Six Sigma Black Belt and the Senior Analyst for MIT Solutions, Inc., developers of analytical and Decision Support tools and systems for healthcare organizations. Frank will use his 30 years of consulting experience for hospitals, large and small medical practices and other health care professionals to CMGMA for a Revenue Cycle Analysis.
The idea of managing the revenue cycle in a medical practice seems pretty basic on the surface. But dive down into the details of the different processes involved and you can soon be looking at dozens and dozens of different steps involving many different people and departments. The fact is, the hardest part about managing the revenue cycle is understanding how it works. To accomplish this, we need to get a handle on where the revenue cycle begins, where it ends and what happens in between. Using cutting edge process management and improvement tools, this workshop guides the attendee through the entire revenue cycle, beginning with developing a defensible fee schedule through learning how to build process and value stream maps of the patient visit cycle and the billing cycle, finishing with a lesson on how to conduct a reimbursement validation and managed care contract value analysis.

| CT General Assembly 2006 Session Bills that Died in Committee | An Act Concerning Outpatient Surgical Facilities (Senate Bill No. 621). This bill would have required the Department of Health to adopt regulations by October 1, 2006, establishing a standard set of exemptions for single specialty outpatient surgical facilities from licensure and certificate of need requirements.
An Act Concerning Surgical Services (Senate Bill No. 647). This bill would have clarifed the scope of oversight by the Office of Health Care Access (OHCA) regarding outpatient surgical facilities.
An Act Concerning Cooperative Health Care Arrangements and Standards in Contracts Between Health Insurers and Health Care Providers (Senate Bill No. 670). This bill would have (1) permitted health care providers to enter into cooperative arrangements that would be exempt from certain antitrust laws (subject to approval by the Attorney General), and to require managed care organizations to negotiate in good faith with providers who participate in such arrangements, and (2) established standards for contracts between health insurers and health care providers. The bill would have set out the standards that the Attorney General was to consider in deciding whether to approve or disapprove a cooperative arrangement.
An Act Concerning Cooperative Health Care Arrangements (Senate Bill No. 28). This bill would have allowed health care providers to enter into cooperative agreements not subject to antitrust laws, and to require managed care organizations to negotiate in good faith with such providers. This bill was substantially similar to the first part of Senate Bill 670 (discussed above).

| CT General Assembly 2006 Session Enacted Legislation | Public Act 06-28 - An Act Concerning Certificate of Need Capital Expenditure Thresholds This act increases the certificate of need threshold for capital expenditures from $1,000,000 to $3,000,000, and for major medical equipment from $400,000 to $3,000,000.
Public Act 06-108 - An Act Concerning Extended Reporting Period Coverage Under Medical Malpractice Insurance Policies. This act revises the conditions under which malpractice insurers must make extended malpractice insurance coverage available to insured providers, in one instance by limiting the conditions under which malpractice insurers must such coverage available, and in another instance broadening the class of insured providers who are entitled to such coverage.
Public Act 06-110 - An Act Concerning The Supervision of Physician Assistants. This bill revises the supervision requirements for physician assistants (PAs) by (1) making a distinction between supervision in a hospital versus other settings and (2) eliminating a requirement that the supervision in any setting be at the specific location where the PA is practicing.
Public Act 06-178 - An Act Concerning Standards in Contracts Between Health Insurers and Physicians. This act requires each contracting health organization (managed care organization (MCO) or preferred provider network) to implement a procedure by October 1, 2007 under which a contracted physician, physician group, or physician organization may view the fee schedule that determines the payment amount for the most commonly performed and billed services. The act also establishes a task force to study contracts between insurers and physicians to determine if legislation is needed to address certain contractual issues.

| President Signs Legislation Delaying Medicare Cuts for 6 Months | On Saturday, Dec. 29, the president signed the Medicare, Medicaid and SCHIP Extension Act of 2007, which replaces the 10.1 percent reduction in Medicare Part B payments scheduled for 2008 with a six-month, 0.5 percent increase. Physicians will again face a reduction in Part B payments in July 2008 unless Congress once again intervenes.

| Providers May See Increase in Claims Rejections Beginning Next Week | The Centers for Medicare & Medicaid Services (CMS) recently announced that beginning Sept. 3, carriers will no longer correct billing or pay-to provider information submitted on Medicare claims submitted by group practices. This may lead to an increase in claims rejections when not using the correct National Provider Identifier (NPI) or NPI and legacy number combination. Group practices should be reporting the group NPI or group legacy number in combination with the group NPI in the billing or pay-to-provider identifier filed.

This statement is being submitted on behalf of the members of the Connecticut Medical Group Management Association on Raised Bill 14 – An Act Prohibiting Co-payments for Preventive Care.
Raised Bill 14 prohibits managed care companies from imposing a copayment, deductible or other out-of-pocket expense for preventive care services. This would include charges for annual physicals and periodic health evaluations, including tests and diagnostic procedures ordered in connection with routine examinations such as annual physicals; routine prenatal and well-child care; child and adult immunizations; tobacco cessation programs; and obesity weight-loss programs. As medical practice managers, we believe this to be a good idea as research has shown that increased cost sharing discourages people from seeking necessary care, but we do have one concern. The bill before you does not specify what happens when managed care companies are prohibited from requiring the copayment – does the physician lose that payment and bare the economic brunt or does the managed care company reimburse the physician? We ask that before passing this bill out of committee, you clarify this point. To ask physicians to bare the financial brunt of this bill is unfair given the financial lose that physicians currently take on Medicaid, Medicare and dually eligible patients and even patients who have private insurance whose claims are routinely denied.
We look forward to working with this Committee throughout the 2010 session to help guarantee the best possible health care for all Connecticut patients.

The bill before you would expand professional liability insurance closed claim reporting requirements, grant the Insurance Commissioner the authority to fine entities that fail to submit reports as required and add confidentiality provisions. Our main concern with the bill is its definition of a “closed claim”. The bill defines a closed claim as a claim that has been settled or otherwise disposed by the insuring entity, self-insurer or health care provider, where all indemnity and expense payments have been made. A claim may be closed with or without an indemnity payment to a claimant. We interpret this to mean that a “closed claim” could be the “write off” of a balance due from the patient and that this would then be “reportable”. This is problematic for physician offices as physicians sometimes forgive balances due from patients for reasons unrelated to a professional liability issues.
In recent years, the citizens of the State of Connecticut have faced increasing financial hardship due to rising rates of unemployment, increases in the rate of personal bankruptcy, the rising cost of health insurance as well as the prevalence of high deductible health plans. In this environment it is increasingly common for physicians to forgive patient balances due to a patient’s financial hardship, particularly in the context of a patient suffering from a debilitating or terminal disease. Further, it would be difficult to retroactively determine why a claim was closed.
We are concerned that, as written, the legislation would have the unintended effect of causing physicians to be less likely to forgive balances due from patients who find themselves dealing with financial hardship. While we do not believe that this was the intent of the legislation, it may be the effect. We hope that this Committee will consider clarifying the definition.

| Summary of 2010 Healthcare-Related Bills in the Connecticut General Assembly |
The following are brief summaries of some of the bills before the Connecticut General Assembly for the 2010 legislative consideration. See www.cga.ct.gov for more information or details.
House Bill 5004 - An Act Concerning Transparency in Health Insurance Claims Data. Purpose: To require insurers to disclose to certain employers upon request certain claims data.
House Bill 5258 - An Act Implementing the Recommendations of the Program Review and Investigations Committee Concerning Scope of Practice Determinations for Healthcare Professionals. Purpose: To implement the recommendations of the Legislative Program Review and Investigations Committee concerning scope of practice determinations for health care professionals. The recommendations include the creation of a standardized process with the Department of Public Health to determine scope of practice changes.
House Bill 5235 - An Act Concerning Evidence of Noncoverage of Health Insurance. Purpose: To require insurers and other entities, in the event of a denial based on the reason that the requested service is not a covered benefit, to mail to an insured a copy of the applicable provision of the policy or certificate governing such decision, and to include in such mailing a notice with the contact information of the Insurance Department and the Office of the Healthcare Advocate.
Senate Bill 12 - An Act Clarifying Postclaims Underwriting. Purpose: To distinguish short-term health insurance policies from those having a duration of one year or more for the purpose of the Insurance Commissioner’s approval of rescinding, canceling, or limiting a policy, to limit the scope of an investigation into a suspected preexisting condition and to specify requirements for insurers or health care centers that accept telephonic applications for individual health insurance coverage.
Senate Bill 14 - An Act Prohibiting Co-payments for Preventive Care. Purpose: To promote the health of Connecticut residents by prohibiting co-payments, deductibles, or other out of pocket expenses for preventive care services (i.e., annual physicals, prenatal/well-child visits, immunizations, tobacco cessation, obesity weight loss programs).
Senate Bill 15 - An Act Concerning Prescription Drug Co-payments. Purpose: To require any co-payment required by an individual or group health insurance policy for prescription drugs to be the same regardless of whether such drugs are obtained through a retail pharmacy or through a mail order pharmacy.
Senate Bill 17 - An Act Concerning Health Care Provider Rental Network Contract Arrangements. Purpose: To require listings of covered entities be updated at least every 90 days, to prohibit covered entities from granting subsequent access to third parties to health care services, discounted rates or fees established in contracts between health care providers and contracting entities, and to specify that violations are deemed unfair or deceptive insurance practices under section 38a-816 of the general statutes.
Senate Bill No. 51 - An Act Concerning Machine-Readable Medical Benefits Identification Cards. Purpose: To require the issuance of machine-readable medical benefits identification cards and scanner devices to read or access such cards.
Senate Bill 52 - An Act Concerning Proof of Health Insurance Coverage for Children. Purpose: To reduce the number of uninsured children in this state.
Senate Bill 130 - An Act Concerning Cancelled Doctors’ Appointments. Purpose: To protect patients by requiring physicians and medical practices to post notice relating to missed appointment fees.
Senate Bill 192 - An Act concerning APRNs and Primary Care Providers for Individual or Group Health Insurance Policies. Purpose: To allow an insured or certificate holder to designate a participating, in-network physician or APRN as such insured’s or certificate holder’s primary care provider
Senate Bill 222 - An Act Concerning Complaints Pending in DPH Against Physicians and Certain Other Healthcare Provider and Establishing a Mediation Program for Medical Malpractice Actions. Purpose: To permit a person who files a complaint with DPH alleging incompetence, negligence, fraud or deceit by a HCP to obtain information about the status of the complaint, notice of final disposition of the complaint, and to establish a mediation program for medical malpractice actions
Senate Bill 252 - An Act Concerning Medical Malpractice Data Reporting. Purpose: To expand professional liability insurance closed claim reporting requirements, grant the Insurance Commissioner the authority to fine entities that fail to submit reports as required and add confidentiality provisions.
